Honda underreported 1,729 claims of injuries or deaths since 2003

Among these underreported cases were eight Takata airbag inflator ruptures not submitted. Following an independent audit of its safety reporting procedures, Honda has found massive holes in its methodology and practices that resulted in 1,729 claims of injuries or deaths going unreported to federal authorities dating back to July 2003. The cases should have been submitted to the National Highway Traffic Safety Administration as part of its quarterly Early Warning Reports (EWRs) under the TREAD Act, but they fell through the cracks for a variety of reasons. Honda blames the underreporting on three factors: data entry errors, computer coding problems and "an overly narrow interpretation of what constituted a 'written notice' under the TREAD Act." The first two issues were related to the computer program that collected the claims. If employees didn't enter a date in the "written claim received" field, then they were omitted from the EWRs. Also, the company's internal component codes didn't always match those used by NHTSA, and only the ones that were the same were disclosed. Finally, third-party documents, including police reports, were not considered. Honda says the computer error is now corrected, and the company is updating its data entry training. In the future, written and oral claims will be included in EWRs, as well. Among these underreported cases were eight Takata airbag inflator ruptures not submitted in Honda's EWRs, including one death and seven injuries. However, the automaker claims NHTSA was already aware of all of these incidents either from the agency's own records or from the company's notification outside of the EWR process. Unfortunately, this problem could have been stopped much sooner. The issue was first brought to light in 2011 but didn't result in a followup. NHTSA advised the automaker of discrepancies in January 2012, and it still did nothing. This third-party audit wasn't commissioned until September 2014. "Honda acknowledges that it lacked the urgency needed to correct its problems on a timely basis," it says in the announcement. Separately, the Japanese government is starting an investigation, as well. According to Reuters, the Japanese Transport Minister has created a task force to look into the Takata recalls and find out whether Honda under-reported incidents there. Takata allegedly hid failed airbag rupture tests in 2000

New evidence suggests Takata engineers knew about the dangers of the supplier's ammonium-nitrate-propelled airbag inflators as early as 2000, but employees hid or even destroyed test results. The allegations came out in a pre-trial hearing in a civil suit from a woman who claims that a forcefully deploying airbag in her 2001 Honda Civic paralyzed her. The pre-trial hearing included an examination of a deposition from Thomas Sheridan, a former Takata airbag engineer, to see if the evidence was admissible for the case, according to the New York Times. Sheridan alleged that Takata created a report for Honda in June 2000 that showed the parts failed, but the supplier hid the testing data. The company also reportedly got rid of the ruptured components so that there was no physical evidence. "But when I went to look for the parts, because some of the parts had come apart, they were no longer available. They had been discarded," he said in the deposition, according to the Times. Takata disputes these allegations, and one of the company's lawyers asserts the inflators in the 2001 Civic are safe. "None of them have ruptured, zero," attorney David M. Bernick told the Times. "We have no evidence, in fact we have evidence to the contrary, that this inflator was defective at the time of the accident." However, Honda has recalls for the driver's side airbag in the 2001-2005 Civic. Previous reports also indicated some Takata employees allegedly knew the inflators were dangerous. For example, an investigation by the Wall Street Journal in 2015 cited internal memos from US employees in 2000 that complained that their counterparts in Japan altered or hid the results of failed validation tests. The New York Times also found evidence of engineers joking about manipulating results. The first Takata inflator recall came on Isuzu models in 2001, and automakers have recalled millions of vehicles around the world since then. Several companies, including Honda, have pledged to stop using Takata's inflators, and he US government fined the supplier $70 million last year. Related Video:

Honda withdraws LMP2 prototype from Pikes Peak

Things don't always go as planned, especially when it comes to racing. That's the lesson that Honda had to learn the hard way at Pikes Peak this year. Unfortunately that means that the ARX-04b LMP2 prototype won't be running up the mountain in this weekend's Race to the Clouds. Honda was slated to run its new LMP2 prototype at Pikes Peak, but a series of misfortunes meant that the entry had to be withdrawn. The prototype was set back with a series of mechanical difficulties, chief among them a turbocharger failure that inflicted damage on the vehicle's engine. That left Honda with little choice but to withdraw the entry. "We some had some problems that forced us to miss practice on Tuesday and Wednesday," Honda spokesman Eric Mauk confirmed in correspondence with Autoblog. Given that Justin Wilson – despite his extensive experience particularly in open wheels – is considered a rookie at this event, "race rules state that he must practice all three sections of the race course before the race. With just two days of practice left (you can only practice one section a day due to time constraints), we could not do that, so we had to withdraw." It's an unfortunate turn of events for Honda and Wilson, to be sure, but the manufacturer will still be entering a number of other vehicles at Pikes Peak this weekend. Included among them are the electric CR-Z prototype, and the NSX that is serving as pace car for the event. All told, Honda is fielding 13 entries in 11 different classes.
