Honda underreported 1,729 claims of injuries or deaths since 2003

Among these underreported cases were eight Takata airbag inflator ruptures not submitted. Following an independent audit of its safety reporting procedures, Honda has found massive holes in its methodology and practices that resulted in 1,729 claims of injuries or deaths going unreported to federal authorities dating back to July 2003. The cases should have been submitted to the National Highway Traffic Safety Administration as part of its quarterly Early Warning Reports (EWRs) under the TREAD Act, but they fell through the cracks for a variety of reasons. Honda blames the underreporting on three factors: data entry errors, computer coding problems and "an overly narrow interpretation of what constituted a 'written notice' under the TREAD Act." The first two issues were related to the computer program that collected the claims. If employees didn't enter a date in the "written claim received" field, then they were omitted from the EWRs. Also, the company's internal component codes didn't always match those used by NHTSA, and only the ones that were the same were disclosed. Finally, third-party documents, including police reports, were not considered. Honda says the computer error is now corrected, and the company is updating its data entry training. In the future, written and oral claims will be included in EWRs, as well. Among these underreported cases were eight Takata airbag inflator ruptures not submitted in Honda's EWRs, including one death and seven injuries. However, the automaker claims NHTSA was already aware of all of these incidents either from the agency's own records or from the company's notification outside of the EWR process. Unfortunately, this problem could have been stopped much sooner. The issue was first brought to light in 2011 but didn't result in a followup. NHTSA advised the automaker of discrepancies in January 2012, and it still did nothing. This third-party audit wasn't commissioned until September 2014. "Honda acknowledges that it lacked the urgency needed to correct its problems on a timely basis," it says in the announcement. Separately, the Japanese government is starting an investigation, as well. According to Reuters, the Japanese Transport Minister has created a task force to look into the Takata recalls and find out whether Honda under-reported incidents there. 4.5 million Hondas, Acuras are being recalled worldwide to fix a fuel pump defect

Honda is recalling more than 4.5 million cars worldwide because of a fuel pump problem that could lead to engine failure or stalling in traffic, which could cause an accident. The recall includes 2.54 million vehicles in the United States. The automaker had previously recalled 628,000 U.S. vehicles in 2021 and 136,000 in 2020 for the same issue. Honda issued recalls over the same issue earlier this month in China and Japan. National Highway Traffic Safety Administration recall documents say the impellers inside fuel pumps of 2017-2020 Hondas and Acuras were improperly made, meaning they “can deform and interfere with the fuel pump body” over time. This could prevent the engine from starting, or can cause a stall while driving, which depending on the situation in traffic, could be dangerous. Fortunately, Honda says it hasn't received any reports of crashes or injuries. To remedy the problem, Honda will replace the in-tank fuel pumps, at no cost to owners, in the entire U.S. recall population of 2,539,902 vehicles, though the defect may be present in only 1 percent of those cars. Because of the size of the recall, repairs will be done in phases, with first owners receiving notification letters in February. Dealer notifications began this week. “Countermeasure replacement parts are not available in sufficient quantities for all recalled vehicles at the time of this announcement, and American Honda intends to notify vehicle owners in stages," Honda wrote. Meanwhile, owners wishing to confirm that their vehicle is included can enter their VIN at the NHTSA recall site or at American Honda's and Acura's recall platforms.  NHTSA's campaign number for the recall is 23V858000. NHTSA gives okay for three automakers to skirt tire-related recalls

BMW, Honda, and Mercedes-Benz are all going to avoid small recalls, after the National Highway Traffic Safety Administration issued petitions for "findings of inconsequential noncompliance" to the three manufacturers, according to Tire Business. Basically, the petitions allow the brands to avoid recalls for some very, very minor issues.
BMW had tire placards on 364 X6 M CUVs that stated the car could only handle four passengers, when in reality it had room for three in the back. Actual plausibility of fitting three real humans in the slope-roofed Bimmer aside, the Munich-based manufacturer argued it was inconsequential, as the placards were correct regardless of the number of passengers.
Honda's case focused on 212 2011 and 2012 Acura TSX sedans equipped with 18-inch wheels. The TPMS systems on these cars were set for 17-inch wheels, rather than the larger hoops, but even with the lower settings, the tires maintain adequate load capacity.
